ABOUT THE PROCEDURE

Eyelid surgery is tailored to the needs of each patient and may involve the upper eyelid, lower eyelid, or both. In the upper eyelid, excess skin and protruding fat that create a tired look or limit peripheral vision are addressed. In the lower eyelid, puffiness and dark circles caused by fat protrusion or skin laxity are treated.

In some cases, fat is not simply removed but repositioned to fill hollow areas beneath the eye, such as the tear trough, rather than being fully excised. This approach helps achieve a more natural, balanced transition between the eye and cheek.

CONTRAINDICATIONS

Ocular surface problems or significant dry eye need treatment first.
Uncontrolled medical conditions affect anaesthesia safety or wound healing.
Expectations assume a dramatic change in eye shape rather than a natural refinement.
Noticeable eyelid ptosis is present and requires careful assessment to determine whether additional muscle repair is needed.
There are visual concerns that require an ophthalmologic evaluation before surgery.

Assessment and planning

The upper and lower eyelids are examined to assess the amount of excess skin, fat distribution, strength of the eyelid-lifting muscle, and their impact on vision and appearance. This determines whether treatment is needed for the upper eyelid, lower eyelid, or both.

Upper eyelid surgery

  • Local anaesthesia with sedation or general anaesthesia is used depending on the case.
  • A precise incision is made in the natural eyelid crease to conceal the scar.
  • Excess skin is removed, and protruding fat is reshaped or removed.
  • In cases of muscle weakness, additional repair may be performed to improve eye opening.
  • The incision is closed with fine, dissolvable sutures over several days.

Lower eyelid surgery

  • The incision may be made from inside the eyelid (no visible scar) or beneath the lashes when excess skin needs to be removed.
  • Protruding fat is repositioned into hollow areas beneath the eye to improve the transition between the eyelid and cheek.
  • Excess skin is removed or tightened when needed.
  • Incisions are closed with fine sutures or left without sutures if made internally.

After surgery

  • Mild swelling and bruising around the eyes are expected during the first days.
  • Temporary dryness or slightly blurred vision may occur.
  • Cold compresses are recommended, along with avoiding eye rubbing or excessive visual strain early on.
  • Follow-up visits monitor healing, swelling, and symmetry between the eyes.

RESULT

A clear improvement in eye openness and freshness is visible after surgery, with swelling and bruising gradually resolving over the following days and weeks. When upper eyelid skin previously obstructed vision, peripheral vision may improve. In the lower eyelid, puffiness and dark circles decrease, and the transition between the eye and cheek becomes smoother.

The goal is a more youthful, refreshed appearance that preserves natural eye expression, not a change that makes the eyes look unfamiliar. Scars are typically concealed within the eyelid crease or beneath the lashes.
