ABOUT THE PROCEDURE

Fat grafting, also called fat transfer, combines two aesthetic goals in one procedure: contouring an area where unwanted fat is present and using that purified fat to enhance volume in a selected recipient area. Fat may be collected from the abdomen, waist, flanks, or thighs, then carefully processed before being placed according to an individualized treatment plan.

The procedure does not rely on implants or synthetic fillers. The final outcome depends on the quality and amount of available fat, the way it is processed, how it is distributed, and the characteristics of the recipient tissue. Some transferred fat is naturally absorbed during the first months, so treatment planning focuses on a balanced long-term result rather than temporary overfilling.

Have localized donor fat that can be harvested safely from areas such as the abdomen, waist, or thighs.

Are in stable general health and have a reasonably stable weight.
Have suitable skin and tissue quality in the intended recipient area.
Understand that some fat absorption is expected and final volume develops gradually.

CONTRAINDICATIONS

There is insufficient donor fat because of very low body fat or limited donor areas.
Weight is unstable or significant weight loss or gain is expected soon.
Uncontrolled medical conditions, bleeding disorders, or healing concerns increase surgical risk.
The patient smokes and cannot follow the required cessation plan before surgery.
There is an active infection, unexplained lump, cyst, or unresolved concern in the donor or recipient area.
Expectations assume that no transferred fat will be absorbed after treatment.

Assessment and planning

The process begins with assessment of the donor areas and the area to be enhanced. At Z Clinic, high-frequency aesthetic ultrasound is used during this phase to evaluate fat thickness, tissue quality, and underlying structures in both donor and recipient zones. The surgeon evaluates available fat, skin quality, existing contour, and the realistic amount of volume that can be achieved. The plan focuses on proportion and harmony, not simply adding volume, and uses ultrasound findings to guide safe, precise fat placement.

Fat harvesting and processing

  • Fat is collected from selected areas through small access points using fine cannulas.
  • The harvested fat is purified and separated from blood, fluid, and material not suitable for transfer.
  • The purified fat is prepared for placement while preserving its quality as much as possible.

Fat placement

  • Purified fat is placed in carefully selected points and tissue layers for even distribution.
  • The technique considers gradual fullness and smooth transition between treated and surrounding tissue.
  • Appropriate cannulas are used to distribute fat precisely and reduce the likelihood of irregularity or lumpiness.
  • Fat is positioned in multiple layers, including subcutaneous, intramuscular, and, where appropriate, supraperiosteal planes, to achieve smoother distribution and more natural integration with the underlying anatomy.
  • Ultrasound is used during injection to confirm depth and plane of placement in real time, enhancing safety and accuracy.

Postoperative phase

  • Compression garments are used in the donor areas as directed.
  • Direct pressure on the treated recipient area may need to be avoided for a surgeon-defined period.
  • Temporary swelling and bruising can occur in both donor and recipient areas and settle gradually.
  • Follow-up appointments monitor healing, fat retention, and the evolution of the final contour.

Scars

Fat grafting leaves only hidden scars. Small access points for cannulas are placed in discreet, naturally concealed locations such as skin folds, the lower back, groin crease, or around the hips, and fade progressively over time.

RESULT

Initial improvement is visible after treatment, but the final result takes time because of normal swelling and the natural absorption of some transferred fat during the first months. Once tissues settle, the fat that successfully integrates remains and the contour becomes more natural and balanced.

Fat grafting is not intended to create excessive volume or radically alter body proportions. It is designed to add measured fullness and improve harmony. Stable weight and adherence to postoperative guidance support fat retention and long-term results.
