ABOUT THE PROCEDURE

A back lift addresses loose skin and folds that may develop in the upper or lower back, especially after substantial weight loss or natural loss of skin elasticity over time. When the main concern is excess skin, liposuction alone is often not enough because fat removal does not correct skin laxity.

The treatment plan depends on where the laxity is located. Surgery may focus on the upper back along the bra line, the lower back and flanks above the buttocks, or a more extensive area in cases of severe skin excess. The aim is to improve back and waist contour while preserving natural transitions between body areas.

INDICATIONS

Have excess or loose skin on the back, especially after major weight loss.
Have visible folds in the upper or lower back when wearing clothing.
Feel that the back contour remains uneven despite stable weight and exercise.
Have lower-back and flank laxity above the buttock area.
Experience irritation, discomfort in skin folds, or difficulty finding comfortable clothing.
Are in good general health and maintain a reasonably stable weight.
Understand that surgery improves back contour but leaves scars that can be placed in less visible clothing lines where possible.

CONTRAINDICATIONS

The concern is localized fat only with good skin elasticity, as liposuction may be sufficient after assessment.
Weight is unstable or significant additional weight loss is expected.
Uncontrolled medical conditions, bleeding disorders, or wound-healing concerns increase surgical risk.
The patient smokes and cannot follow preoperative and postoperative cessation requirements.
Expectations assume a scar-free back or an exaggerated result that does not match natural body proportions.
There is an active skin infection or inflammatory condition in the back area.

Assessment and planning

The back is assessed from top to bottom to evaluate fold location, amount of excess skin, fat distribution, skin quality, and the relationship between the back, waist, and buttocks. The surgeon determines whether an upper, lower, or more extensive back lift is required and plans the incision placement to be as discreet as possible under clothing.

During surgery

  • The procedure is generally performed under general anaesthesia.
  • A horizontal incision is made in the upper or lower back according to the area of laxity; severe cases may require an additional incision based on the surgical plan.
  • Excess skin and fat are removed in carefully planned amounts.
  • Liposuction may be used in selected areas to refine back contour when appropriate.
  • Supportive tissues are tightened to smooth the back shape and reduce visible folds.
  • Incisions are closed with fine sutures, and drains may be used in selected cases.
  • Compression garments support the tissues during recovery.

After surgery

  • Swelling, bruising, and back tightness are expected early in recovery.
  • Sleeping on the stomach or side is often advised to avoid direct pressure on the back.
  • Compression garments are worn as directed to reduce swelling and support tissue healing.
  • Gentle movement returns gradually, while overhead arm movement, heavy lifting, and strenuous exercise are avoided initially.
  • Follow-up appointments monitor wound healing, swelling, back symmetry, and scar maturation.

RESULT

Back contour becomes smoother after surgery, but final results develop gradually as swelling decreases and tissues settle. Over the following months, skin folds and excess tissue become less noticeable, and the back appears more balanced with the waist and buttocks.

The goal is a firmer, more proportionate back contour, not excessive tissue removal or an unnatural change in body shape. Stable weight and proper scar care support longer-lasting results.
