ABOUT THE PROCEDURE

A scar cannot be completely erased, but in many cases it can be improved so that it becomes less visible, smoother, and better aligned with the natural lines of the skin. Scar revision may involve removing the old scar, releasing tight tissue, and carefully closing the skin to reduce tension and improve healing.

Scars vary widely. They may result from previous surgery, injury, burns, deep wounds, or past inflammation. For this reason, no single technique suits every scar. The treatment plan depends on the age, location, width, thickness, tension, relation to muscles or joints, surrounding skin quality, and the patient’s personal healing tendency.

INDICATIONS

Have a wide, irregular, raised, or visibly prominent scar.
Have a depressed or tight scar that affects skin texture or movement.
Have a scar that lies in an unfavorable direction relative to natural skin lines.
Experience discomfort, tightness, pain, or persistent itching related to a scar.
Have a scar from surgery, trauma, injury, or a previous wound that affects the appearance of an area.
Have allowed enough time for the original scar to heal and stabilize before evaluation.
Understand that the goal is meaningful improvement, not complete scar removal.

CONTRAINDICATIONS

The scar is still new and undergoing normal healing changes.
There is an active infection or inflammation in the scar or surrounding skin.
There is a strong tendency toward hypertrophic scarring or keloids without an appropriate prevention plan.
Uncontrolled medical conditions, bleeding disorders, or wound-healing concerns increase surgical risk.
The patient smokes and cannot follow cessation requirements before and after surgery.
Expectations assume complete removal of the scar or no visible evidence of surgery after treatment.

Assessment and planning

The scar is examined for its type, direction, width, thickness, and degree of tension. The surrounding skin, previous surgical history, and past healing patterns are also evaluated. The surgeon then determines whether surgical revision is appropriate or whether adjunctive treatment may be needed later.

During surgery

  • Anaesthesia is selected according to scar size and location; this may be local anaesthesia, local anaesthesia with sedation, or general anaesthesia.
  • The old scar may be excised, or tight tissue around it may be released according to the treatment plan.
  • The scar may be redirected to follow natural skin lines or modified to distribute tension more effectively.
  • Tissue layers are closed carefully with fine sutures to minimize surface tension and support healing.
  • Specialized dressings or scar-support measures may be used depending on the treatment area.

After surgery

  • Mild swelling, tenderness, and temporary redness around the revised scar are expected.
  • The wound should be kept clean and dry, following all instructions for dressings and sutures.
  • Avoiding tension, friction, and direct sun exposure supports better scar healing.
  • Postoperative care may include silicone products, scar massage, or additional treatment depending on medical assessment.
  • Follow-up appointments monitor healing and the gradual improvement of the scar.

RESULT

The revised scar begins a new healing process after surgery and normally passes through phases of redness and firmness before gradually improving. Final scar appearance often takes several months to stabilize, with continued improvement in color and texture over a longer period.
